2. You agree and affirm that the information you give Sports Traveler as part of the enrollment is true and correct. Failure to provide true and correct information may result in the termination of your membership or revoking of any Rewards Points issued to your account.
 

3. Sports Traveler will credit Rewards Points to your account based upon the dollar amount of your purchases from Sports Traveler. You will receive one (1) Rewards Point for every dollar spent (excluding shipping fees and service fees) on the SportsTraveler.net website. The Rewards Points will be applied to your Rewards account only after the purchase is completed, full payment is received and you have attended your event. Points will become visible in your on-line account within 30 days of the completion of the event you purchased. Corporate group proposals, group trips, custom travel packages, private home rentals, and custom event tickets not listed on www.SportsTraveler.net website are not eligible for rewards points. Also, shipping costs are not calculated into your points total. Sports Traveler reserves the right to change or modify the manner in which points are assigned to your account.
 

4. To obtain Rewards Point credit for your purchases, you must be logged into your account or include your Rewards program account number/id with your order.
 

5. Rewards Points will be credited only to the account provided. The account name must match billing address of the person making the reservation and/or purchase from Sports Traveler.
 

6. If you find that your account has not been properly credited, you must notify Sports Traveler in writing within fourteen (14) days of the purchase for which you seek Rewards Points. If written notice is not received within 14 days, Sports Traveler will not adjust your account.
 

7. Your Rewards Points will not expire and will continue to accumulate as you make additional purchases on the SportsTraveler.net website.
 

8. Sports Traveler may terminate your account if you have earned no Rewards Points for a period of 12 consecutive months.
 
 

9. Past Clients: Clients that made travel package purchases on www.SportsTraveler.net between January 1, 2002 and May 19, 2009 are eligible for a portion of up to 25% of those total purchases (excluding shipping costs) to be applied to their rewards account as points. Clients must fill out this form to enroll in the program and will be notified via email of the number of points they are eligible to redeem. Past corporate and/or group orders, ticket orders, hotel room only reservations, and private home rentals are not eligible to be redeemed for past customers. If past customer points are awarded, they will appear in the customer account as "inactive" points. Inactive points will be activated when the customer makes their next purchase with Sports Traveler. All membership terms then apply.
 
 

10. Your Sports Traveler Rewards Points cannot be redeemed for cash. The Rewards Points can only be redeemed to obtain Rewards listed in the Rewards Catalog on the Sports Traveler website.
 
 

11. Sports Traveler reserves the right to add or withdraw available Reward items, change the number of Rewards Points required for redemption of a Reward item, substitute items of like or similar quality, limit the number of Reward items available to you, or add new Reward items at any time without notice to you.
 

12. You agree that you will be responsible and pay any taxes that may be due and owing as a result of redeeming Rewards Points.
 

13. You understand and agree that the Rewards have been furnished to Sports Traveler by an independent third-party vendor. SPORTS TRAVELER MAKES NO REPRESENTATION OR WARRANTY AS TO THE MERCHANTIABILITY OR FITNESS FOR ANY PURPOSE OF THE REWARDS AND EXPRESSLY DISCLAIMS ANY LIABILITY WHATSOEVER FOR ANY LOSS, DAMAGE, INJURY OR DEATH THAT MAY DIRECTLY OR INDIRECTLY ARISE OUT OF OR IN CONNECTION WITH THE USE OF THE REWARDS ITEM. SPORTS TRAVELER IS FURNISHING THE REWARDS ITEM ON AN AS IS BASIS.
 

14. You agree that Sports Traveler may share, rent, transfer and/or sell the information you provide as part of the enrollment process with other parties who may or may not be affiliated with Sports Traveler.
 

15. Sports Traveler reserves the right to amend, suspend, or terminate this Rewards program at any time.
 

16. Any dispute regarding the terms and conditions of Sports Traveler Rewards must be resolved in Cook County, Illinois, where you agree jurisdiction and venue are proper.
 

17. This agreement is to be interpreted using the laws of the State of Illinois, without reference to its conflicts of law provisions.
